麻豆区 is vitally concerned with the health and well-being of its students. We know insurance can be confusing, so this site provides valuable information and tools for students, parents and medical providers.


United HealthCare Student Plan: Calvin鈥檚 health insurance for students

The鈥疭tudent Health Insurance Plan (SHIP) (United HealthCare Student Plan)鈥痮ffered through 麻豆区 is a plan designed with a student鈥檚 needs and budget in mind and covers all care received at Calvin Health Services, for both undergraduate and graduate students. Most importantly, SHIP gives students access, via referrals, to an extensive network of health care providers practicing in the Grand Rapids area.

All registered degree-seeking domestic and international students are required to enroll in the student health insurance plan unless

  • an international student has proof of full sponsorship by a government agency including health insurance听
  • a domestic student has proof of health insurance provided by a United States employer or state or federal agency (ACA compliant plan).听

Only students meeting these specific criteria may request to waive out of the student health insurance plan.

The deadline to request a waiver ends 30 days before the start of classes. The premium of the student health insurance plan is assigned to your student account. The student health plan is annual if it begins in the fall semester and becomes active on August 15th. Students starting in the spring have active insurance starting on January 1st. Students graduating in December will be registered for the fall-only plan.听

Other cooperating insurance

Whether students have the Student Health Insurance Plan (SHIP) or request a waiver because they are covered through their parent鈥檚 insurance, on-campus health care at Health Services is available to鈥痑ll鈥疌alvin students, regardless of what insurance they carry. 听See below for important information on requesting a waiver.

Most鈥痩ocal insurance carriers鈥痗ooperate with Calvin Health Services regarding medical claims but many insurance plans from outside of Michigan consider our practice 鈥渙ut-of-network鈥. 听We bill directly to most out-of-state insurance plans. We do not bill Medicare, out-of-state Medicaid, or subsidized Michigan Medicaid plans. 听See our list of participating insurance companies 禄

麻豆区 requires all full-time students to enroll in the Student Health Insurance Plan (SHIP) OR request a waiver. Waiver requests are only accepted during the open enrollment period.听

Activate your student health insurance OR request a waiver during the open enrollment period. *Note -- the Student Health Insurance Plan (SHIP) becomes active on August 15th or January 1st, depending on which semester the student is enrolling at Calvin. This happens automatically if a request to waive the student health insurance plan was not requested during the open enrollment period.

For Fall Semester: Open Enrollment Period is July 1st through July 31st

For Spring Semester: Open Enrollment Period is November 1st through December 10th.

Insurance Waiver and Insurance Enrollment Form

Domestic students may request a waiver if they can provide proof of ACA compliant insurance (see below for criteria ). Most domestic students are under their parent鈥檚/guardian鈥檚 insurance plan until the age of 26. In general, these employer funded plans meet this requirement. Government funded plans such as Medicaid or Medicare from outside of the state of Michigan are only accepted if the Medicaid/Medicare plan covers emergencies in the state of Michigan.听

Any domestic student may request a waiver if:听

(a)听听听听The student provides proof of active health insurance provided by their United States based employer (for students employed in the US while attending Calvin)听
(b)听听听听The student provides proof of active insurance under parent鈥檚/guardian鈥檚 employer plan (up to age 26, with proof of dependent coverage)听
(c)听听听听Marketplace plans through the state or federal exchange听
(d)听听听听Depending on your state, you may qualify for Medicaid. Go to https://www.healthcare.gov/.gov to determine if you may be eligible. Waiver requests must be submitted with proof of activation of the Medicaid Plan, or Calvin鈥檚 Student Health Insurance Plan (SHIP) will be activated.

International students who have full financial sponsorship by a government agency including health insurance must submit a copy of their financial guarantee to health@calvin.edu. A financial guarantee that covers the current academic year must be on file before a waiver can be requested. Waivers are applied annually.听

What are the criteria to be eligible to request a waiver? 听Any international student may request a waiver if:听

(a)听听听听The student provides proof of full sponsorship by a government agency including health insurance, or
(b)听听听听The student provides proof of active health insurance provided by their United States employer (only applies to students employed fulltime in the USA, while attending Calvin), or
(c)听听听听The student provides proof of active insurance under parent鈥檚/guardian鈥檚 United States employer (up to age 26, with proof of dependent coverage when parents are employed within the USA)

All other international students are required to enroll in Calvin鈥檚 Student Health Insurance Plan (SHIP). Any individual insurance purchased by international students will not be considered for waiver requests and will be considered secondary insurance to the student health plan. Prior to beginning a waiver request, the student should review all the criteria and must meet one or more of the above criteria. To initiate the waiver request the student must:

1.听听听听Have a Health insurance card
2.听听听听Have Health insurance plan description or brochure听
3.听听听听Know their 麻豆区 Student ID number听
4.听听听听Submit a waiver request during the open enrollment period.

International students who have applied for waivers in previous semesters will need to apply again..

Insurance Waiver and Insurance Enrollment Form

The Student Health Insurance Plan (SHIP) premium will be assessed on your 麻豆区 student account in early August. Refunds are only permitted within 30 days of enrollment, and once the plan is activated (used for a medical expense) it cannot be refunded. 听To avoid any fees or a Calvin class registration hold, turn in waiver request or enrollment form prior to the OPEN ENROLLMENT PERIOD deadline. For those enrolling later than usual, the premium will be assessed to your account 7-10 days after you have met with your academic advisor and have enrolled in your courses.


Fees at Health Services

麻豆区 maintains a fully staffed comprehensive primary care office on campus for all students, including billing and payment options for in-network and out-of-network patients. 听The cost of care is similar to the cost of care in the local community. Any fees not covered by the insurance plan are the responsibility of the patient and expected to be paid in full at the time of service by after insurance has paid their portion. Cash, checks and credit cards are accepted for non-students.听

Copays, Co-insurance, and Deductibles

Patients are responsible for copayments, deductibles and co-insurance as indicated on the insurance explanation of payment. 听听
鈥⑻齋tudents: We will charge the balance due to your student account after your insurance company has processed your claim.听
鈥⑻鼺aculty/staff: We charge your HSA or credit card on file after the insurance has processed your claim.听
鈥⑻齌he Calvin Health Services accepts cash, Visa/Mastercard and HSA/FSA cards. Please call 616-526-6187 ext 4 to make a payment with your credit card.听
鈥⑻齌he student鈥檚 Workday account will be billed for the balance of the cost of care. Patients can review a detailed claims receipt with an explanation of charges in the Health File. Deductibles and co-payments are applied to the student鈥檚 Workday account or posted to the credit card/HSA card on file. 听
鈥⑻齌o review a detailed history of your claims from Health Services: yourhealthfile.
鈥⑻齀t is the patient鈥檚 responsibility to confirm coverage with his or her insurance plan prior to their appointment.

Laboratory Services听

Calvin Health Services contracts with Corewell Health Laboratory as a reference lab for all tests that cannot be performed at Calvin. Corewell Health will bill your insurance directly. You will receive a statement from Corewell Health for the patient responsibility portion of your laboratory charges. This is separate from any charges you may receive from Calvin Health Services. The Student Health Insurance Plan (SHIP) covers all labs drawn on campus even when they are sent to Corewell Health. 听

Pharmacy

Calvin Health Services providers offer to may dispense 听a prescription medication stocked in the office. The university's Student Health Insurance Plan (SHIP) covers all prescription medications and most over-the-counter medications dispensed from Calvin Health Services. Calvin Health Services offers competitive self-pay pricing for most prescriptions and over-the-counter medications for students receiving care at Calvin Health Services even when they do not have the university鈥檚 Student Health Insurance Plan (SHIP).听
