Health Coverage Tax Documents

麻豆区 will no longer be mailing Form 1095-C to former employees due to a change in the IRS rule in December 2022. We will continue to file the forms directly with the IRS for all individuals covered in the 2024 calendar year.

If a former employee wants a copy of Form 1095-C, they can request one by calling the Human Resources office at 616-526-6495, by emailing the Human Resources office at humanresources@calvin.edu, or by mailing a request to 麻豆区 Human Resources, 3201 Burton St SE, Grand Rapids, MI 49546.
